Tom has spent much of his working life in adult social care.  From delivering care and support to the frail, elderly and physically challenged to deputising and supporting the manager of a care home for adults with mental health conditions.  Tom has keen interests in mental health and learning difficulties, particularly men's mental health.  Tom previously worked for the PCN as a Care Co-ordinator but joined the Social Prescribing Team in December 2023.

Outside of work Tom enjoys a relaxed lifestyle with his dog, Duke.  You will often find Tom travelling or finding an adventure with a backpack and a tent.  Tom is also a keen cook and enjoys making a mess in any kitchen!

Tom supports St Chads, Midsomer Norton and Chilcompton Surgeries.

Matt joined the PCN Team in August 2023, having previously worked in South Glos. Council's housing department and NHS Mental Health Services for the previous 10 years.  As a Social Prescriber Matt supports patients with social or non-clinical needs such as emotional/mental health support, housing, benefits and financial issues and social isolation.

Outside of work Matt is generally found playing the drums and guitar, cooking or camping.
